PPO: Preferred Provider Organization

Copay: A copay is a fixed out-of-pocket amount paid by an insured individual for covered services (one-time payment per visit/service/Rx cost).

EPO: Exclusive Provider Organization; in-network only

Deductible: The amount you pay for covered health care services before your insurance plan starts to pay (one amount per calendar year—not per incident or service).

HDHP: High Deductible Health Plan

HSA: Health Savings Account

Coinsurance: Percent of expenses patient is responsible for paying after deductible

EOB: Explanation of Benefits

Out-of-Pocket Maximum: Most one will pay for covered services during a policy year

Premium: Cost to purchase insurance coverage.
